RESEARCH PLAN	This completed form is required for ALL projects. Type or print all information requested. Every question must be answered.
RESEARCH PLAN (TEAMS)	This completed form is required for ALL TEAM projects. Type or print all information requested. Every question must be answered.
APPROVAL FORM	This completed form is required for ALL projects.
REGISTERED RESEARCH, INSTITUTIONAL/INDUSTRIAL SETTING	This form must be completed by the scientist supervising the student research conducted in a registered research institution (e.g., university lab, medical center, NIH, SSTP, etc.) or industrial setting.
QUALIFIED SCIENTIST	Required for research involving animals, controlled substances and pathogens; may be required for rDNA, tissues, and humans. Must be signed prior to the start of student experimentation. Click HERE for tips on avoiding common errors in completing this form.
DESIGNATED SUPERVISOR	Required if the Qualified Scientist is unable to supervise the experiment OR for projects using hazardous materials or devices. Click HERE for tips on avoiding common errors in completing this form.
HUMAN SUBJECTS	Required for all research involving humans. IRB approval required before experimentation. Click HERE for tips on avoiding common errors in completing this form. (The Adult Sponsor, Qualified Scientist, or Designated Supervisor cannot serve on the SRC/IRB for their particular project.)
INFORMED CONSENT	Recommended for all projects involving human subjects, required for all involving risk. Use a separate form for each test subject.

HUMAN AND ANIMAL TISSUE	Required for all projects using viable fresh tissue, organs, human or animal parts, including blood, blood products, teeth, primary cell structures, and body fluids (plant tissue is excluded). (SRC approval required before experimentation.)
